Enrollment

Our registration team is ready to assist you throughout the enrollment process and address any questions or concerns you may have.  Here are some tips to follow when completing the application:

• Only legal guardians may complete the enrollment form.

  • If the applicant is 18 years of age and considered an adult.  

  • They will need to complete the enrollment form and sign documents. 

  • They will also need to provide proof of residency in their name.  

  • Their information will also go under “contact”.

Proof of Age

Please provide an official document showing proof of age. Acceptable documents are as follows:

• Birth Certificate

• Passport

• Visa

• Driver's License State ID

Proof of Residency

• Proof of residency must be in the legal guardian’s name.

• The student must live at that residence.

• It needs to be out of the envelope, where the parent's name(s), current address, and date within the past 60 days can be seen.

Other Documentation

• You must provide custody documents if you identify someone other than the individual(s) listed on the birth certificate as a legal guardian.

• Immunization: You can complete an exemption form if you do not have access to up-to-date immunizations.

Previous School Information

• Please complete previous school information: last day attended, school name, and the city and state of the school.

• Must indicate whether or not the student has been expelled.

Application Notes

• Enter the anticipated start date.

• Please make sure you are entering the date of birth, address, and the student’s legal name correctly.

• We require 3 contacts (parent 1, parent 2, contact 2, and contact 3), each with three different phone numbers.

• Please ensure you are signing the application's Home Language Survey and Medications portion correctly with the parent/legal guardian’s name.
